Application for Guyana Passport.

• Two completed application forms (a four-page document) can be downloaded from the internet: www.minfor.gov.qy On white Office Paper 216 mm x 330 mm OR 8 x 13 inches
• The form must be signed by a Recommender-page 3, Reference # 7
• The signature of the applicant must not exceed rectangle page 2 and on page 3 #6.
• Two passport size photographs (not more than 1 month old) (45 x 35mm or 4 cm x 5 cm), white background. The recommender is required to endorse on the reverse side of the photographs with the words: "I certify that this is true likeness of Mr., Mrs. or Miss. .." and add his or her signature.
• Two coples of birth certificate
• Two coples of previous passport (data pages),
• Two coples of DEED POLL If there is a change in name
• If applicant is a married woman, the following must be provided:

1. Two copies of marriage Certificate (must be officially translated in the English language)
2. -Two coples of spouse's birth certificate and passport

In the case of Lost or Stolen Passport: Applicant must fill page 4 Information Sheet and submit a Police Report.

Requirements for Passport sized Photographs

• All applicants must ensure that both identical passport size photographs meet the following specifications:
• Applicant must be fully covered/properly fully clad.
• Dimensions of the photograph meet the requirements as stated on page one of the Application Form (minimum 26x32mm, maximum 35x45mm).
• Clarity and contrast are correct, proper exposure (no washout).
• No shadows or glare from improper lighting, glasses or jewellery.
• Background is plain white (no colour or shadows).
• Neutral expression (No smile, open mouth or frown).
• No head piece or hair covering (except those worn for religious purposes & only if the
• full facial features are clearly visible).
• No sunglasses.
• Eyes, Ears, Full Facial Features are clearly visible.
• Studs/Earring/s not accepted on male applicants pictures.
• Photos are stamped with photographer stamp and dated
• Stamp and date must be legible (not smeared).
• No glare/shine.
• The reverse side of one photograph is endorsed by the recommender with the words: "I certify that this is a true likeness of..." and his or her signature is affixed (only)
• N.B. Photographs that do not meet the above mention specifications will not be accepted.

Acceptable Recommenders

While page 1 of the Passport Application Form states that the recommender should be Guyanese, this requirement is not applicable for Guyanese who are resident overseas. However the recommender must be professionally known to the applicant for over two (2) years.

Below are some examples of suitable recommenders:

• Certified Accountant (with Business Card or License No.)
• Business Manager/Human Resources Manager
• Doctor/Dentist/Registered Nurse (Reg.)
• Imam/Pandit/Priest (with Lic or Business Card/Office Stamp)
• Lawyer/Notary Public/Justice of Peace
• Pharmacist (with Lic) Professional Engineer (with Lic or Business Card)
• Police Officer (with Badge)
• School Principal (with Office Stamp/Card)
• University/College Professor(with Office Stamp
